Welcome!

We’re open for new patients to register with us. Before filling in the online form, please check if your address is in our catchment area using the map below.

We are aware that the search bar and “click to return to catchment” buttons may not work on some Microsoft Edge browsers. But it usually does work on Google Chrome. We are unable to fix this as a third party built and operates the map.

Use the Register with a GP Surgery online tool. It makes it easier for you to register with us than the older paper form. It also helps us do our work faster. This service is NHS built and operated, so your personal information is safe.

You don’t need to show proof of address, immigration status, ID, or an NHS number.

If you need a paper form, it is still available. You can download the paper form here (PDF) to print and fill in at home if you prefer.

Completing registration

We’ll need to check your identity and address to finish your registration. Please bring the following to the surgery:

Proof of identification

  • This must be a current form of ID with a recent photo of you.
  • We CAN accept: your NHS medical card, passport, or driving licence.
  • We CAN’T accept: student cards or national insurance number cards.

Proof of address

  • This must be a recent utility bill in your name.
  • We CAN accept: gas bills, electricity bills, council tax bills, or water bills.
  • We CAN’T accept:
  • mobile phone bills
  • landline phone bills
  • bank statements
  • credit card statements
  • utility bills in the name of a parent or relative who is not registered with the practice.
  • If you don’t yet have the right proof of address, we can accept a copy of your tenancy agreement or rent book. It must be in your name and not in the name of a relative.

To register children under 16, at least one parent or legal guardian must also be registered with us.

It is usual to see a GP before your registration is complete.

Named GP

We register all our patients with a named GP but patients can see any GP in the practice.

Data Sharing

If you have chosen to ‘opt out’ of sharing your data at your last surgery, you may need to fill out new forms. For more details, please see our patient data policy.

Vaccination Records

If you are currently registered in the UK, we should get your vaccination records from your current GP. But if you haven’t registered with an NHS GP before, please provide a copy of all your vaccinations.
